RESUMEN

Introdução: O uso de membranas de barreira de exclusão de epitélio para regeneração óssea guiada tem sido apresentado na literatura como abordagem de tratamento bem sucedida, essas barreiras irão impedir a incorporação de células de tecidos moles no enxerto ósseo ou no es paço criado pelo defeito e permitir que apenas as células osteogênicas estejam presentes. Relato de caso: Trata-se de um caso clinico de um paciente que evoluiu com perda óssea significativa por lesão Endo-periodontal levando a perda dentaria, para a reabilitação com implantes se fez necessário uma reconstrução de um defeito crítico com a utilização de tela de titânio para arcabouço ósseo de mistura de enxerto autógeno e biomaterial e recobrimento com membrana de colágeno, o enxerto autógeno foi removido de área doadora na mandíbula e utilizado de forma particulada. Conclusão: As telas de titânio apresentam viabilidade e previsibilidade no aumento ósseo vertical, horizontal e tridimensional em decorrência da sua ampla aplicabilidade, elasticidade, plasticidade adequadas e boas propriedades mecânicas. Avanços na diminuição de suas taxas de exposição e risco de complicações no período de cicatrização melhoram a cada dia a previsibilidade da técnica, bem como a associação com outros materiais como os hemoderivados... Introduction: The use of epithelial exclusion barrier membranes for guided bone regeneration has been presented in the literature as a successful treatment approach, these barriers will prevent the incorporation of soft tissue cells into the bone graft or the space created by the defect and allow that only osteogenic cells are present. Case report: This is a clinical case of a patient who evolved with significant bone loss due to an endo-periodontal lesion leading to tooth loss, for rehabilitation with implants it was necessary to reconstruct a critical defect using mesh titanium for bone framework of mixture of autogenous graft and biomaterial and covering with collagen membrane, the autogenous graft was removed from the donor area in the mandible and used in a particulate form. Conclusion: Titanium meshes are viable and predictable in vertical, horizontal and three-dimensional bone augmentation due to their wide applicability, adequate elasticity and plasticity, good mechanical properties. Advances in reducing their exposure rates and risk of complications during the healing period improve the predictability of the technique every day, as well as the association with other materials such as blood products... Introducción: El uso de membranas de barrera de exclusión epitelial para la regeneración ósea guiada se ha presentado en la literatura como un enfoque de tratamiento exitoso, estas barreras evitarán la incorporación de células de tejido blando al injerto óseo o el espacio creado por el defecto y permitirán que solo presencia de células osteogénicas. Reporte de caso: Este es un caso clínico de un paciente que evolucionó con una pérdida ósea importante debido a una lesión endo-periodontal que lo llevó a la pérdida de un diente, para rehabilitación con implantes fue necesario reconstruir un defecto crítico utilizando malla de titanio para armazón óseo de mezcla de injerto autógeno y biomaterial y cubriendo con membrana de colágeno, el injerto autógeno se extrajo del área donante en la mandíbula y se usó en forma de partículas. Conclusión: Las mallas de titanio son viables y predecibles en el aumento óseo vertical, horizontal y tridimensional debido a su amplia aplicabilidad, adecuada elasticidad y plasticidad, buenas propiedades mecánicas. Los avances en la reducción de sus tasas de exposición y riesgo de complicaciones durante el periodo de cicatrización mejoran día a día la predictibilidad de la técnica, así como la asociación con otros materiales como los hemoderivados... Platelet concentrates are used for cell induction and stimulation in tissue repair processes. The aim of the present systematic review and meta-analysis was to compare the biological and cellular properties of advanced platelet-rich fibrin (A-PRF) to those of other platelet concentrates. Searches were conducted on the PubMed/Medline, Scopus, Web of Science, Embase and LILACS databases using a search strategy oriented by the guiding question. A total of 589 records were retrieved. Seven articles of in vitro experimental studies were selected for qualitative data analysis and four were selected for meta-analysis. The release of growth factors, distribution of cells in the fibrin membrane, and cell viability, the fibrin network, and fibroblast migration were investigated. In the final analysis, statistically significant differences were found for the A-PRF group with regard to platelet-derived growth factor, transforming growth factor, epidermal growth factor and vascular endothelial growth factor at all assessment times. A difference was found with regard to bone morphogenetic protein only in the later assessment, and no differences among groups were found with regard to platelet-derived growth factor or insulin-like growth factor. The results of this systematic review and meta-analysis suggest that A-PRF has superior cellular properties and better release of growth factors compared to other platelet concentrates.

BACKGROUND: The length of hospital stays of patients undergoing orthognathic surgery depends on related local and systemic factors. Hematological changes resulting from orthognathic surgery, followed up in the postoperative period until hospital discharge, have neither yet been established for specific cases, nor for normal situations. This study aimed to describe the hemodynamic parameters of patients undergoing orthognathic surgery, considering a prediction of hospital discharge. A prospective analytical study was carried out on a sample of convenience for this purpose. METHODS: The erythrogram, leukogram, and platelet count were assessed 24 and 72 hours after surgery and compared with preoperative values. Intraoperative volume loss was also assessed. Inferential statistical analyses were performed as analysis of variance or Friedman test, paired Wilcoxon test, Mann-Whitney test, and 2-tailed Spearman correlation. RESULTS: The red blood cell count (mean ±â€Šstandard deviation [median]); (4.60 ±â€Š0.24 [4.56]), hemoglobin (12.82 ±â€Š1.03 [12.75]), hematocrit (39.51 ±â€Š3.47 [39.60]), and red cell distribution with (32.60 ±â€Š0.88 [33.05]) were higher preoperatively (P < 0.001). Mean corpuscular volume (83.87 ±â€Š5.63 [83.10]), mean corpuscular hemoglobin (27.50 ±â€Š1.47 [28.00]), leukocytes (6262.00 ±â€Š1448.36 [6380.00]). The volume loss varied between 463.87 and 752.13 mL (608.00 ±â€Š144.13 [630.00]). CONCLUSIONS: The results of the hematological evaluations corroborate the expectation of standard discharge from the hospital, with reflected changes resulting from volume loss and intense postoperative inflammatory response, even under the anti-inflammatory pharmacological effect.

Introdução: A Fasceíte Necrotizante Cervical é uma infecção rara, geralmente com evolução rápida e progressiva da fáscia superficial e do tecido adiposo subcutâneo. Pode se desenvolver a partir de uma infecção odontogênica que se dissemina para os planos faciais e profundos. O presente trabalho tem por objetivo relatar um caso clínico de uma paciente do gênero feminino, 49 anos de idade, atendida na Emergência do Hospital Getúlio Vargas-PE, com quadro clínico sugestivo de Fasceíte Necrotizante Cervical de origem Odontogênica. Relato de caso: O tratamento proposto foi antibioticoterapia parenteral de amplo espectro, desbridamento cirúrgico radical com instalação de dreno de penrose transfixante submandibular direito, múltiplas exodontias para remoção do foco primário da infecção, além de curativos orientado pela equipe de infectologia. Considerações finais: Excepcionalmente, as Fasceítes Necrotizantes Cervicais devem ter diagnóstico e manejo imediato, devido aos altos índices de destruição, rápida progressão e mortalidade. Nos casos de Fasceíte Necrotizante de origem odontogênica, a antibioticoterapia, remoção do(s) focos(s) de infecção, desbridamento cirúrgico e monitoramento intensivo constituem a terapêutica adequada... Introduction: Cervical Necrotizing Fasciitis is a rare infection, usually with rapid and progressive evolution of the superficial fascia and subcutaneous adipose tissue. It can develop from an odontogenic infection that spreads to the facial and deep planes. This study aims to report a clinical case of a 49-year-old female patient, seen at the Emergency Department of Hospital Getúlio Vargas-PE, with a clinical condition suggestive of Cervical Necrotizing Fasciitis of Odontogenic origin. Case report: The proposed treatment was broad-spectrum parenteral antibiotic therapy, radical surgical debridement with the installation of a right submandibular transfixing penrose drain, multiple extractions to remove the primary focus of the infection, in addition to curatives guided by the infectology team. Final considerations: Exceptionally, cervical necrotizing fasciitis must have an immediate diagnosis and management, due to the high rates of destruction, rapid progression and mortality. In cases of Necrotizing Fasciitis of odontogenic origin, antibiotic therapy, removal of the foci (s) of infection, surgical debridement and intensive monitoring constitute the appropriate therapy... Os lipomas são as neoplasias mesenquimais mais frequentes do corpo humano, sendo raros na cavidade oral. No aspecto histopatógico, são constituídos basicamente de adipócitos maduros. Clinicamente, apresentam-se como massas nodulares, sésseis ou pedunculadas, de consistência amolecida, aspecto gelatinoso e de superfície lisa; frequentemente são assintomáticos e sem ulcerações. O presente artigo tem por objetivo relatar um caso de um lipoma na região do ventre da língua, que impedia a correta adaptação da prótese total inferior, além de trazer transtornos fonéticos à paciente, tendo sido proposto um tratamento cirúrgico através de uma biópsia excisional da lesão... Lipomas are the most common mesenchymal neoplasms of the human body, being rare in the oral cavity. In histopatoógico aspect, consist primarily of mature adipocytes.
